Hexarelin, also called examorelin, is a chain of six amino acids made in a laboratory. It is a close relative of GHRP-6, with one link altered. It works on the same receptor the body's hunger hormone uses, which is a different receptor from the one sermorelin and tesamorelin use. What sets it apart from the rest of this group is that researchers found it also sticks to a protein in heart tissue, and studied what happens there.

In rat cardiac membranes, hexarelin was shown to bind CD36. In isolated perfused hearts, activating CD36 with hexarelin raised coronary perfusion pressure in a dose-dependent way, an effect absent in animals lacking CD36. The authors of that work suggested CD36 may mediate the coronary vasospasm seen in hypercholesterolaemia and atherosclerosis.
