The follistatin gene is spliced into more than one form. FST344 is the one whose precursor is 344 amino acids long, and once the signal peptide is cut off the mature protein it secretes is 315 residues, usually written FS-315. FS-288 is a shorter isoform. A 2006 study compared them directly: they bound activin about equally well, but differed sharply in how strongly they attach to cell surfaces, with FS-315 lowest of the three. In one assay FS-288 suppressed an activin-driven response while FS-315 increased it. Papers often say only "follistatin", so which isoform was used is worth checking before comparing results.

Not as a protein. Studies in mice, in monkeys and in people delivered a follistatin gene inside a virus so that muscle cells produced follistatin themselves. Three small human trials of the FS344 gene, all from the same research group, have reported outcomes. In the Becker muscular dystrophy and inclusion body myositis trials, two of six participants each showed little or no change on the six-minute walk test, and the myositis trial measured its patients against eight untreated matched subjects rather than a randomized control group. The third, in Duchenne muscular dystrophy, enrolled three of six planned patients, and none of the three increased their walk distance. The myositis trial also drew a published letter disputing its functional claims, with a reply from its authors; both are cited in the Follistatin 344 research article.
