What is ACE-031?
ACE-031 is not a peptide. It is a fusion protein. One part is the piece of the human activin receptor type IIB that normally sits outside the cell. That is joined to the tail section of a human antibody. Its international non-proprietary name is ramatercept. The receptor fragment still grabs the proteins the real receptor would have caught, so it works as a decoy, mopping them up before they reach muscle. One of those proteins is myostatin, sold separately here as GDF-8. Follistatin 344, also sold here, blocks the same protein another way.

Is ACE-031 a peptide?+−
No. ACE-031, international non-proprietary name ramatercept, is a dimeric fusion protein of about 60,000 g/mol. It joins the extracellular ligand-binding domain of the human activin receptor type IIB to the Fc region of human IgG1. It has no CAS registry number, which is expected for a protein of this kind.
Why was development of ACE-031 discontinued?+−
A randomised, placebo-controlled trial was stopped after its second dose level. The published report gives the reason as potential safety concerns of epistaxis and telangiectasias, meaning nosebleeds and small dilated blood vessels. The same report states that no serious or severe adverse events occurred and that the efficacy signals it recorded were trends that did not reach statistical significance. What did the 2025 analysis of black-market ACE-031 find?+−
Fourteen products sold under the ACE-031 name were analysed. Two contained no protein reactive to an ACVR2B antibody. The other twelve contained the full-length human activin receptor IIB rather than the Fc fusion protein the name refers to, which the authors confirmed with a protease that cleaves Fc fusions and could not cleave the products. Published results obtained with characterised ACE-031 are not evidence about material of that kind.
What is the difference between ACE-031, GDF-8 and Follistatin 344?+−
GDF-8, or myostatin, is the ligand: it limits muscle growth. ACE-031 is a soluble decoy receptor that traps it before it reaches the receptor. Follistatin 344 is a binding protein that also binds it. A decoy receptor and a binding protein are different mechanisms, so the three are sold and studied separately and results reported for one are not reported for another.
