It carries a positive charge and two aromatic rings. It builds up inside mitochondria, the parts of the cell that make most of its energy. There it sticks to cardiolipin, a fat found only in the inner membrane of those parts.

Why do published studies call this elamipretide?+−
SS-31, elamipretide, Bendavia, MTP-131 and Ocuvia are all names for one compound. SS-31 is the laboratory designation used in the preclinical mechanism work. Elamipretide is the international non-proprietary name and is what every clinical trial used. Bendavia and MTP-131 were development names, and PubChem treats them as synonyms rather than separate entries. Searching only "SS-31" will return the mechanism papers and miss the trials. The Published Research tab states which name each cited study used.
What did the clinical trials find?+−
Mixed, and the larger and better-controlled the trial, the less it found. A single four-hour infusion in people with reduced heart pumping reduced ventricular volumes at the highest dose; a later trial giving it daily for four weeks found no change in the same measurement. In Barth syndrome, the randomized half of a trial missed both primary endpoints and the open-label half that followed reported improvements. In primary mitochondrial myopathy, a 218-person phase 3 trial missed both primary endpoints. Each of these is set out with its numbers on the Published Research tab.
